Kindly be informed that even if you delete your old MySJ account, once you verified the same IC/ Passport and name you will be able to see your vaccination and dependent timeline page again in the new MySJ account.

Once the status reverted to assessment status, kindly update the pregnancy/ breastfeeding link.

Kindly proceed to your profile page and click on the refresh button. You will be able to view your digital certificate at the bottom of the profile page.

Kindly ensure the IC/PASSPORT in the MySJ application matches the IC/Passport in the vaccine card. If both IC matches, your record may not be reflected yet as the PPV has not updated the details in the system. Kindly allow for up to 2 weeks for the health facility to upload your details into the system. Once done, you will be able to view the vaccine appointment details in the MySJ vaccine timeline.

My appointment in the MySJ app and the website shows two different dates. Kindly refer to your MySJ app for the latest appointment. Kindly proceed to check your current status of the vaccine timeline by following the steps below.

  • Click on the ‘COVID-19 Vaccination’ icon on the homepage
  • Choose ‘Vaccine for [your name]’
From here, you will be able to see the timeline of your vaccination status.

Kindly proceed to your profile page and click 'Refresh'.

  • Under your profile tab, scroll down and search for your dependent's name
  • Click on your dependent's name
From here, you will be able to see your dependent's vaccination digital certificate.

Please be informed that you will not be able to see the details of the old appointment in the MySJ app 48 hours after the cancellation. You will be re-enrolled into the vaccination program according to your eligibility by phase, and a new appointment will be allocated to you as soon as new capacity becomes available at the location requested.

Please be informed that "Require Pre-vaccination Assessment" means during the consent stage at PPV, the person in charge in PPV may identify that you may not be suitable for vaccination or etc. You will require to go to a doctor and get a PRE-VACCINATION ASSESSMENT (PVA) done. Then, you will get a new appointment. You are required to bring that form when attending the new appointment.
Note: Pre-vaccination assessment (PVA) also means that you will only get vaccinated in the hospital PPV.

Kindly be informed that registration of liability may be cancelled by the owner of the MySejahtera account. Please click on "Add Vaccine Dependent" and choose "Remove vaccine dependent". Once removed, the dependents may proceed to verify their name and IC number in their own account.

  • Please be informed that only low risk, contact casual no symptoms and contact casual with symptoms (if tested negative for COVID-19) are allowed to enter a Vaccination Administration Centre (PPV).
  • Your MySejahtera risk status message should always be referred to before you decide to leave your home. This is still applicable when preparing to travel to a PPV for vaccination.
  • You will not be allowed to enter if your risk status is HIGH. This is done to minimise the risk of infecting others. Please be patient, your vaccination appointment will be rescheduled to a later date.
