To receive respective appointment details for Phase 1 , kindly
  1. Click 'COVID-19 Vaccination' icon in the homepage
  2. Click 'Vaccine for [your name]' to validate your name and IC/passport number
  3. Answer the vaccination consent assessment

Selected frontliner will receive push notification.Once you receive the push notification,kindly proceed to complete the eligibility questionnaire. Then, the appointment details will be shown for your reference.

Frontliner who havent got selected, please make sure the verified name and IC/passport number are correctly entered. You will be notified soon once you are enrolled for the COVID-19 vaccination.

  1. If you are selected, you will be given a push notification through the My Sejahtera App
  2. Please click on the notification and answer the screening question as soon as you receive the notification Please click on the notification and you will be able to view your appointment details. Alternatively, you can click on the ‘Vaccination’ icon in the homepage, and click on ‘Vaccine for [your name]’ to view your details
  3. Kindly confirm if you will be attending the appointment through the hyperlink. Reminders will be sent to your 3 days before your appointment
  1. Please disregard your appointment and be informed that you may decline your appointment prior to the given date.
  2. Upon declining your appointment, kindly note that you will be given a new appointment date.
Changing of vaccination location is not allowed. If you have moved to a different state from the location that you have registered for vaccination, kindly contact the Vaccine Hotline number at 1800-888-828.
A new appointment date will be given to you at a later date.
Currently, change of vaccination date is not allowed. If you are unable to attend, kindly reject your appointment and you will be given a new appointment date.
If you and your dependents belong in the same phase, MOH will try to accommodate appointments at the same date and location, but this is not guaranteed.
Vaccinees are allowed to proceed for dose 2 if AEFI is not severe. In the event there is severe AEFI, vaccination location will be changed to hospital.

You will receive an appointment notification immediately upon setting the date. Notifications regarding appointments will be given within 14 days prior to the appointment date

If you have registered your dependent as a main user in MySejahtera , you won’t be able to add them as ‘dependant’.A user ID can only be tagged to single IC.
Yes, foreign housekeepers can be taken into account as dependents if they do not have the MySejahtera application.
So far the policy is based on the one decided by the COVID-19 Immunisation Action Body (CITF),the detergence of vaccination dates depends on the qualifying phase. Any policy changes will be notified later.

However if you are selected for vaccination phase 2 and appointment has been assigned along with location of the health facility (PPV-Pusat Penerimaan Vaksin), and if you need to change your address, please contact Talian Pendaftaran Vaksinasi COVID-19 Kebangsaan @ 1800-888-828. They will register your change request and reassign new appointment within 48-72hrs.
