Correct name and IC/ Passport

Kindly make sure your name and IC/ Passport number has been entered correctly in both MySejahtera application and the health facility. Please be informed that for walk-in / online registered user, the status will be updated once we receive the details from the health facility.


Incorrect name and IC/Passport

Kindly refer to the below step to update your name, IC/ passport.

  • Login to your MySejahtera account
  • Click on “MySejahtera” icon
  • Click on “HELPDESK” icon
  • Click on “START”
  • Click on the option C. I want to update my Name, IC/ Passport , User ID or delete my account
  • Click on the option A “I want to update my name & IC number/Passport”
  • Enter your USER ID details and click “NEXT”
  • Enter your USER ID & EMAIL and confirm EMAIL and click on “SUBMIT”

If you’re unable to login to your MySejahtera account, kindly follow the below steps.

  • Click the “Need Help” icon on the bottom of the main login page
  • Click on “START”
  • Click on the option C. I want to update my Name, IC/ Passport , User ID or delete my account
  • Click on the option A “I want to update my name & IC number/Passport”
  • Enter your USER ID details and click “NEXT”
  • Enter your USER ID & EMAIL and confirm EMAIL and click on “SUBMIT”

Once all the above steps are completed, you will be able to see the reverted vaccine module to edit your personal details within the next 24 hours.

Wrong info in PPV or vaccine date in MYSJ app

Please be informed that for the correction of COVID-19 vaccination details, you are advised to contact your health facility to submit a change request.

Wrong (name & IC/ passport ) details in the digital cert

Please be informed that as the vaccination digital certification is issued by the health facility, you are advised to contact your health facility to submit a change request.

Wrong details in Batch number and the expiry date is blank

For some cases, the batch number and expiry date will reflect within the next day. If it does not reflect after 5 days, kindly contact your health facility for clarification.

Kindly refer to the below step to update your location.
  • Click on the ‘COVID-19 Vaccination’ icon in the homepage
  • Choose ‘Vaccine for [your name]’
  • From here, you will be able to see the timeline of your vaccination status.
  • In the event that you wish to update the assessment or location details, kindly click on the provided link to resubmit your assessment.
  • For MySejahtera user, appointment details such as dates and vaccination center will be shared through the MySejahtera application.
  • Notifications regarding appointments will be given within 14 days prior to the date of appointment.
  • Kindly proceed to your device setting and search for MySejahtera app to enable the push notification.
  • Kindly be informed that the Vaccination program is currently in Phase 2 which is focused on remaining front liners, elderly and high risk group.
  • MOH designated appointments based on user's input during registration and as to the best of their capability. If you are a part of Phase 2 or have answered it correctly, please be patient and wait for your turn. You will be notified soon once you have enrolled for the COVID-19 vaccination.
  • For MySejahtera user, appointment details such as dates and vaccination center will be shared through the MySejahtera application under ""COVID-19 Vaccination"". Notifications regarding appointments will be given within 14 days prior to the date of appointment.
  • Kindly proceed to your device setting and search for MySejahtera app to enable the push notification.

Kindly refer to your MYSJ app for latest appointment. If there is any concerns, you can contact the PPV directly to re-confirm.

Please be informed that you may get the same appointment date as your spouse, provided you are in the same “Phase” category according to the JKJAV website. If yes, you may contact the JKJAV hotline at 1800-888-828 to submit your request.

  • You may indicate a desire to change the date by contacting the National COVID-19 Vaccination Call Center at 1800-888-828.
  • The responding agent will cancel the date that has been given to you in the system.
  • Your new date will not be given by the agent who answered the phone.
  • Your new date will be given through MySejahtera.
  • Click on the ‘COVID 19 Vaccination’ icon in your app
  • Click on ‘Add Vaccine Dependant’

If you have registered your dependent as a main user in MySejahtera , you won’t be able to add them as ‘dependant’. A user ID can only be tagged to single IC.

Yes, foreign housekeepers can be taken into account as dependents if they do not have the MySejahtera application.

No. It is mandatory to have valid travel documents information (IC,Passport) to register in Mysejahtera for vaccination.

The account owner can register for different dependents as long as the dependent is not registered for vaccination in a different MySejahtera account.
